Do not submit your payment until you and I setup and agree on the date and time of your examination.        You can use any major credit card, debit card or green dot pre-paid card. You can obtain a pre-paid card at any Rite Aid or other drug stores. Sorry, no personal checks for any reason can be accepted. All private examinations need to be pre-paid to hold the date and time.  You need to make the payment within an hour of us setup the date and time. No date and time will be held without payment.

If you wish to pay with a credit card/debit/pre-paid card simply put in the amount in the blank space next to the dollar sign ($) and then click on the “continue to a secure payment”. This will take you to a secure encrypted merchants bank website to enter your credit card information. No credit card information is obtained or collected by NY POLYGRAPH for any reason.

If you wish to pay with PayPal, simply click the “PAY NOW” button and put the amount in where it say’s “UNIT PRICE” hot update total. Than fill in the credit information and click on “REVIEW ORDER—click SUBMIT”

· Your payment is not refundable once it is made and the date & time are confirmed

· You fully authorized this payment through your credit card or debit card and give your permission for the funds to be made payable to “NY POLYGRAPH”

· You can not change the date of the examination less than 72 hours before the appointment date already scheduled

· Fully payment is required on ALL private examinations

· A 50% deposit is required from all private companies (full payment if out of NYS)

· The credit card needs to be in your name unless you are using a green-dot pre-paid card

· You fully understand and accept that if you cancel or the person does not take the examination or is unable to take the examination for any reason your payment is not refundable.

· If you don't agree to all the terms do not submit the payment
